Our aim
At 321 Medical supported living, we aim to help people live happy and fulfilling lives and also in achieving their goals. We truly care about the people we support and take pride in the services we offer.
What we do
Our supported living programme includes support for young people, homeless families, individuals with physical disabilities, learning disabilities and people with mental health needs. We also run domestic abuse and support services for older people and those with dementia. We motivate people to do as much as they can for themselves and also help them gain practical and cognitive skills through personalised support. This enables people to move on to live independently or provide permanent accommodation with support, when complete independence isn’t possible.

Our services

DOMESTIC ABUSE
Any incident or pattern of controlling, coercive, physical violence, threatening or abusive behaviour between intimate partners or family members, regardless of gender or sexuality is described as Domestic Abuse.
We provide accommodation, support, advice and protection from domestic abuse. Our services will empower people to live safely and free from fear or harm.​
HOMELESSNESS
We support adults who have experienced homelessness or are at risk of being homeless also who may need to overcome a dependence on addictive substances.

YOUNG PEOPLE
We support young people aged between 16 and 25, by encouraging them to focus on their strengths and build positive support networks to achieve their goals.

Our services include

supported housing services which provide a safe place for young people to live, learn and make the transition to independent adulthood

we offer services to help residents move-on and maintain their own independent accommodation.
Our support is mainly focused on accommodation, work & learning, people & support, health, how you feel, individual choices, behaviour, money, rent and practical life skills.
We tailor our support to individual needs, through a structured care and support plan that allows residents to take control of their lives and work towards achieving their personal goals.

DISABILITY
We operate a flexible and unique services providing disability support like; supported housing with care, respite care and nursing or residential homes.
​
Our support time vary from few hours a week to full-time. We also offer intensive support over a short period of time to help achieve a goal.
​
We also provide tailored support and care for people aged 18 or over with mild, moderate or severe disabilities, including: Autism, Physical disabilities, Cerebral Palsy, Associated Mental Health needs and Behavioral challenges.

MENTAL HEALTH
Our main focus here is on Prevention and Recovery Care & Support (PARCS). Our range of mental health support services include:

We operate two different types of supported living projects:
Long term
Our long-term supported living service users require continuous care and support, in a supported environment. They either hold a license or a tenancy agreement.
Short term
This involves Step-up and Step-down framework.
Our step up services focus on community service users who would end up in hospitals / respite care without intervention. However, the step down services focus on clients who do not need hospital stay although have not fully recovered enough to return home.
For both categories, our PARCS staff assist our clients towards hospital, community, tribunal & CPA appointments visits. We also work closely with the out of hours, HTT (Home Treatment Team) and the Community Team involved in their care.
